依据：《中华人民共和国药品管理法》、《处方管理办法》、《药品说明书和标签管理规定》、《医疗机构药事管理规定》等药事管理法规。

一、超说明书用药的定义

超说明书用药（也称为“药品未注册用法”）是指临床实际使用的药品的适应症、给药方法或剂量不在药品监督管理部门批准的说明书之内的用法，超说明书用药的具体含义包括给药剂量、适应人群、适应症或给药途径等与药品说明书不同。

二、在临床工作中，超说明书用药应具备以下5个条件：

1、在影响患者生活质量或危及生命的情况下，无合理的可替代药品。

超说明用药时，必须充分考虑药品不良反应、禁忌症、注意事项，权衡患者获得的利益大于可能出现的危险，保证该用法是最佳方案。

2、用药目的不是试验研究。

用药目的必须仅仅是为了患者的利益，而不是试验研究，这体现医疗人员的基本职业权利。

3、有合理的医学实践证据。

如有充分的文献报道、循证医学研究结果、多年临床实践证明

及申请扩大药品适应症的研究结果等。

4、经医院药事管理与药物治疗学委员会及伦理委员会批准。

在超说明书用药前，应向医院药事会及伦理会提出申请，由医院药事管理与药物治疗学委员会及伦理

超说明书用药的管理规定（试行）

为加强我院药品临床应用管理、持续改进医疗质量、促进临床合理用药，保障临床用药的安全性、有效性、合理性，依据《药品管理办法》、《医疗机构药事管理规定》、《处方管理办法》、《侵权责任法》、《药品说明书和标签管理规定》等法规，结合我院实际情况制定本办法。

1、超药品说明书用药 是指临床实际使用药品的适应证、给药方法或剂量不在国家批准的药品说明书之内，包括年龄、给药剂量、适应人群、适应证、用药方法或给药途径等与药品说明书中的用法不同的情况，又称超范围用药、药品未注册用法或药品说明书之外的用法。

2、临床医生应当根据医疗、预防、保健需要，按照诊疗规范和药品说明书中的药品适应证、药理作用、用法、用量、禁忌、不良反应和注意事项等开具处方。超说明书用药必须是为了患者的利益，而不是试验、研究；或其他关乎医生自身利益的使用，而且必须保证利益大于可能出现的危险。

3、当某种疾病治疗需要使用超药品说明书的药品时，临床科室应提出申请，并提供权威的文献依据（如治疗指南、专家共识、循证医学依据等），提交药事管理与药物治疗学委员会审核、医务部备案，备案材料应包括申请内容，申请人，申请科室意见，药事委员会的讨论意见及结论。必须尊重患者的知情权，
