建筑企业安全生产许可证申请条件和材料

申请条件:

(一)建筑施工企业取得安全生产许可证，应当具备下列安全生产条件: 1、建立、健全安全生产责任制，制定完备的安全生产规章制度和操作规程; 2、保证本单位安全生产条件所需资金的投入;

3、设置安全生产管理机构，按照国家有关规定配备专职安全生产产管理人员; 4、主要负责人、项目负责人、专职安全生产管理人员经建设主管部门或者其他有关部门考核合格;

5、特种作业人员经有关业务主管部门考核合格，取得特种作业操作资格证书; 6、管理人员和作业人员每年至少进行一次安全生产教育培训并考核合格; 7、依法参加工伤保险，依法为施工现场从事危险作业的人员办理意外伤害保险，为从业人员交纳保险费;

8、施工现场的办公、生活区及作业场所和安全防护用具、机械设备、施工机具及配件符合有关安全生产法律、法规、标准和规程的要求;

9、有职业危害防治措施，并为作业人员配备符合国家标准或者行业标准的安全防护用具和安全防护服装;

(二)企业主要负责人、项目负责人、专职安全生产管理人员考核合格人员的人数要求:

1、企业主要负责人包括企业经理(或主持工作的副经理)、分管安全生产工作的副经理(持 A 证);

2、项目负责人人数最少应当达到建筑业企业资质等级要求的项目经理人数(持 B 证)，且必须持相应资质等级的注册建造师证;

3、专职安全生产管理人员人数由以前应当达到项目经理人数的 80% ，增至 100 ,;

4、三类人员数量要求参见资质标准中《建筑施工企业安全生产许可证人员配备表》。

申请材料:

一、延期申请的企业(不包括重新核查企业)根据闽建建,2010,61号文件要求，向省住房和城乡建设厅提交以下材料:

1、《建筑施工企业安全生产许可证申请表》。《申请表》由企业如实在“系统”中填报生成并打印，经企业法定代表人签字加盖公章，并对其真实性负责。

2、建筑施工企业工商营业执照副本、资质证书正副本、安全生产许可证复印件。

3、初审部门对“三类人员”核实情况的书面确认函或企业“三类人员”安全生产考核合格名单及证书复印件。

1、企业法人营业执照复印件;

2、企业主要负责人、项目负责人、专职安全生产管理人员安全生产考核合格名单及证书，数量应当符合相关规定;

3、本企业所有特种作业人员名单及操作资格证书。由企业如实报送名单并提交相关材料，企业需出具承诺文件，承诺提供了完全的人员名单，并经法定代表人签字和加盖本企业公章;

4、本企业管理人员和作业人员年度安全教育培训材料(包括企业培训计划、培训考核记录);

5、本企业申请安全生产许可证前半年内其中一个月的人员工伤保险证明(含有个人社会保障代码、缴费基数、缴费额度、缴费期限等信息的工伤保险人员明细表和缴费凭证);

6、本企业所有施工起机械设备的检验检测合格证明。由企业如实报送施工起机械设备清单并提交相关材料，企业需出具承诺文件，承诺提供了完全施工起机械设备清单，并经法定代表人签字和加盖本企业公章;

7、按规定应接受安全生产评价的建筑施工企业，应当提交安全生产评价机构出具的安全生产评价报告。

3、设置安全生产管理机构及配备专职安全生产管理人员的文件(包括企业设置安全管理机构的文件、安全管理机构的工作职责、安全机构负责人的任命文件、安全管理机构组成人员明细表等);

4、职业危害防治措施(针对本企业业务特点可能导致的职业病种类制定的预防措施);

5、危险性较大的分部分项工程及施工现场易发生重大事故的部位、环节的预防监控措施和应急预案(根据本企业业务特点，详细列出危险性较大分部分项工程和事故易发部位、环节及有针对性和可操作性的控制措施和应急预案);

6、生产安全事故应急救援预案(应本着事故发生后及时有效救援原则，列出救援组织人员名单、救援器材、设备清单和救援

（一）变更企业名称应提交以下材料：

1、企业注册地设区市建设行政主管部门关于同意企业更名的公函； 2、企业名称变更申请报告；

3、已变更的企业法人营业执照复原件印件； 4、企业修正后的章程复印件； 5、对原企业债权债务的承诺书；

6、已变更的企业资质证书正、副本原件及复印件。 （二）变更企业主要负责人应提交以下材料：

1、企业申请变更的文件报告，须经企业注册地设区市建设行政主管部门签署意见； 2、已变更的企业法人营业执照复印件；

3、任命文件或董事会决议；

一．操作人员必须持证上岗，并了解门吊构造，严格按规定进行操作。新造或移

位后的门吊设备，在开始吊重前应进行试吊。

二．操作中发现电气绝缘不良，电压不稳，钢丝绳有损伤，机械运转不灵、电机

设施温度超过规定等异常现象时，应及时停机修理，故障未经排除不得作业，严

禁边操作边修理保养。

三．听从指挥信号，起、落钩要稳、轻、准。垂直起吊、高度以能越过障碍为度，
