四川省省级安全生产文明施工标准化工地

评审办法

第一章 总则

第一条 为了更好地贯彻落实“安全第一，预防为主、综合治

理”的安全生产方针，提高建筑施工安全生产管理水平和文明施工水平，根据《中华人民共和国建筑法》、《建设工程安全生产管理条例》等有关安全生产法律、法规要求，结合本省实际，更好的体现省级安全生产文明施工标准化工地样板示范作用，制定本办法。

第二条省级安全生产文明施工标准化工地（以下简称“安全文明标化工地”）评审依据为《建筑施工安全检查标准》JGJ59-99，以及国家和地方有关标准、规范规程。其施工过程安全生产管理和文明施工应达到省内先进水平，具有较好的经济效益和社会效益，是我省建筑施工安全最高奖。 第二章 预报程序及条件

第三条 四川省省级安全生产文明施工标准化工地采用预报制度，预报工程项目需在工程开工条件审查达到安全生产文明施工标准化要求后，由工程总承包企业填写《四川省省级安全生产文明施工标准化工地预报表》（附件1）及创省级安全生产文明施工标准化工地工作计划，经工程所在地的市、州建设行政主管部门同意后，报省建设工程质量安全监督总站备案。

第五条 预报工程应达到以下规模 （一）市、州人民政府所在地工程项目 1、单体住宅工程4000平方米及以上； 2、单体公共建筑及综合楼8000平方米及以上； 3、单体工业建筑8000平方米及以上； 4、1.5万座（含1.5万座）以上的体育场； 5、2000座（含2000座）以上的体育馆； 6、1000座（含1000座）以上的游泳馆、艺术馆； 7、500座（含500座）以上的影剧院（或多功能厅）；

8、单体建筑面积2000平方米（含2000平方米）以上的市政桥梁； 9、工程投资规模在人民币1000万元以上； 10、具有代表性的标志性工程。

8、单体建筑面积2000平方米（含2000平方米）以上的市政桥梁； 9、工程投资规模在人民币500万元以上； 10、具有代表性的标志性工程。 第三章 工程复查程序及条件

第六条 预报工程在主体完工后进入装饰工程以前报送复查

申请材料，经当地建设行政主管部门签署推荐意见后，报四川省建设工程质量安全监督总站。

一、复查申请材料总目录

二、《四川省省级安全生产文明施工标准化工地预报表》复印件及《四川省省级安全生产文明施工标准化工地复查申请表》（附件2）。

三、工程立项计划书，施工许可证，施工企业资质等级证书，企业安全生产许可证、企业安全生产业绩评价手册、承包合同、工程现场管理人员具备相应的管理资格证书复印件。

四、工程概况和介绍工程安全管理情况的文字材料。 五、如在安全方面获得本地区有关专业部门颁发的地区奖，提供获奖证明复印件一份。

六、能反映施工现场安全文明施工的有关照片或录像带。 七、当地建设行政主管部门按照《四川省建筑施工现场监督检查暂行办法》在各阶段检查时的评价情况及推荐意见。

第八条 四川省建设工程质量安全监督总站在接到申请复查材料后，组织有关安全技术管理人员、专家组成复查小组，对该工程施工现场安全生产条件进行复查。

一、听取推荐单位和申报单位对工程安全文明施工的情况介绍。

二、查阅安全文明施工的有关资料，以及采用新技术、新工艺、新材料的情况。

三、检查“安全生产文明施工措施费”的使用情况。 四、查看现场，并按照《建筑施工安全检查标准》JGJ59-99对施工现场评分。

第四章 工程审批与发证

第十条 工程经复查后，由复查小组写出书面复查报告， 提交四川省建设工程质量安全监督总站审查，通过后上报省建设厅审批。

第十一条所有经复查的工地应按照国家有关规范标准继续组织施工直至竣工。在此期间，如该工程项目降低安全生产条件，发生重伤以上安全事故或重大质量事故的，将取消评比资格，并立即收回“争创省级安全生产文明施工标准化工地项目”铜牌。

第十二条 工程竣工验收后，且综合评价为优良，符合条件的工程由总承包单位填报《四川省省级安全生产文明施工标准化工地

第十三条 每隔半年，省建设厅将行文公布获奖单位名单，并通报表彰，同时，按照《四川省建筑施工企业安全生产许可证动态管理暂行办法》的规定，对以上获得表彰的企业给予30分的加分。有关地区、部门和获奖企业可根据本地区、本部门和本企业的实际情况，对获奖企业和有关人员给予精神和物质上的奖励。各地建设工程招投标部门应对获奖施工企业在投标中加分或其它方法予以奖励。 第六章 附则

第十四条 本办法由省建设厅负责解释。

第十五条 本办法自颁发之日起实施，关于实施《四川省省级安全生产文明施工标准化工地评审办法》的通知（川建厅质安发[2000]0304号）文件同时废止。
